What are the responsibilities and job description for the Social Worker position at Skilled Nursing Care?
Actively seeking Director of Social Services for our skilled nursing facility. The purpose of this position is to provide services to meet social and emotional needs that affect the residents in a ability to achieve their highest level of function.
Responsibilities include but not limited too:
-This person will participate in the development of residents' comprehensive care plans.
-Initial and psychosocial assessments for all residents.
-Facilitate the discharge process for our short-term residents to ensure safe discharge back to the community.
-Provide ongoing support for residents and their families.
-Participate in the MDS process with a special focus on the IDCP team.
Qualifications:
-LSW in the State of NJ
-Long-term experience of at least 1 year preferred
-Working in the field of Social Work for at least 1-3 years preferred
Salary : $45,000 - $60,000
