A Skilled Nursing Facility is looking for an MDS Coordinator in Orange County, NY.

Job Responsibilities :

  • Conducting thorough assessments and care planning for residents
  • Developing and implementing care plans and related activities
  • Preparing and distributing care plan calendars on a regular basis
  • Keeping care plans and assessments current and up-to-date
  • Reviewing and ensuring clinical records are accurate and complete
  • Reporting ongoing issues with documentation to the Director of Nursing
  • Upholding strict confidentiality regarding resident and facility information

Qualifications :

  • Previous experience with MDS 3.0 is required
  • Active RN license in New York State
  • Experience in long-term care is essential
  • Strong organizational and professional skills
  • Proficient in computer systems and technology
  • Excellent written and verbal communication abilities
  • Ability to work collaboratively in a team environment with a positive attitude
