Part-Time MDS Coordinator – Skilled Nursing Facility (Manchester, NH Area)

Responsibilities:
- Complete and manage MDS assessments in accordance with federal and state regulations
- Ensure accurate and timely documentation to maximize reimbursement
- Collaborate with interdisciplinary teams to develop individualized care plans
- Provide support and guidance on MDS processes across multiple facilities as needed
Qualifications:
- Active RN or LPN license in NH
- Minimum 1 year of MDS 3.0 experience in a long-term care setting
- Knowledge of PDPM, RAI guidelines, and CMS regulations
- Strong organizational and communication skills
- Ability to travel within the Manchester, NH area as needed
Salary;
LPN: $31 - $37/Hour
RN: $42 - $48/ Hour
