What are the responsibilities and job description for the Mobile Wound Care NP (Pinetop/Show Low) position at Skilled Wound Care Professionals?
Job Summary: Our Mobile Wound Care Nurse Practitioner will work as a member of the practice to provide wound care to patients within assisted living, nursing homes, long-term care facilities, and private homes with or without home health's staff. This position is responsible for providing wound management for wounds collaboratively with the facility wound nurse/nursing team and home health.
Primary Duties and Responsibilities:
- Traveling to scheduled locations weekly for wound rounds at various homes or facilities in a region that is specific to your preference.
- Performing bedside wound care skills including: wound assessment, debridement, dressing application, pressure relief, pain/odor control, and minor surgical procedures and application of biologics when necessary.
- Providing education to the patient, family, facility staff, and home health agency staff to promote understanding and healing, and for the management of patient’s wounds
- Develop and manage an individualized wound care treatment plan for optimal healing outcomes or chronic maintenance
- Collaborates closely with facility wound nurse and nursing staff on wound management practices
- Knowledge of up-to-date topical wound care products and therapies, including ordering, application, and management of the products on wounds and ostomies, also including negative pressure wound therapy
- Orders, reviews, interprets, and integrates diagnostic studies and laboratory findings for initiating or modifying the treatment plan
- Collaborate with the patient’s primary care provider when comorbidities are contributing to or affecting the healing of a wound to determine a comprehensive plan
- Update the primary care provider on the status and management plan of the wound as appropriate
- Prescribes medications and/or durable medical equipment to assist in healing of wounds
- Provides the required billing information for services provided by the Wound Care NP and maintains the documentation necessary for those services billed
- Reviews patient prior medical records and maintains their current medical record related to the wound diagnosis and contributing comorbidities
- Patient chart encounters will be closed according to the standard of care
- Assist with the identification of wound care quality measures, and provide data and documentation for tracking of quality measures
- Participates in professional development activities and maintains professional affiliations.
- Completes required number of continuing medical education (CME) hours for license and certification
- Attends required meetings or committees as requested
- Maintains patient confidentiality
- Performs other duties as determined collaboratively with team and administrators that are within the scope of practice of the Wound Care NP
- Maintains a copy of all professional records including current licensure, certification, continuing education, and other appropriate records to practice

EXPERIENCE / EDUCATION / CERTIFICATION REQUIREMENTS:
- Graduate of an accredited Nurse Practitioner school
- Licensed by the Arizona Board of Registered Nursing and in good standing for NP
- DEA License
- BLS and CPR certified
- Adult/geriatric NP or family medicine NP
- Wound care: 2 years (Required)
- Ability to react calmly and effectively in emergency situations
- Ability to interpret clearly and establish/maintain effective working relationships with patients, medical staff, and facilities
- Work well independently in addition to being skilled in interdisciplinary collaboration
- Is a self-starter, motivated, and demonstrates positive attitude
- Valid and unrestricted driver’s license
PHYSICAL REQUIREMENTS:
- Limited bending turning, kneeling and stooping, lifting, sitting and standing
- Repetitive motion required including computer entry
- Must be able to drive and carry a backpack with basic medical equipment and a computer
Job Types: Part-time, Contract
Pay: $90.00 - $120.00 per hour
