Sky View Rehabilitation and Health Care, a 192-bed Skilled Nursing Facility located in Croton-on-Hudson, NY. Sky View serves as the bridge between hospital and home, spanning the gap from recovery to community reentry. Our luxurious and hotel-like environment is the ideal setting for those in need of Post Acute Rehabilitation or Long-Term Care. Our team of highly skilled physical, occupational, speech, and respiratory therapists really care and work together to create an individualized therapy plan that helps residents reach their highest level of functioning in as short an amount of time as possible – preparing for their eventual return home.
Social Worker (Job Summary)
- The social worker plays a vital role in supporting the emotional, psychosocial, and discharge planning needs of sub-acute residents.
- Conducting comprehensive psychosocial assessments upon admission and updating them regularly.
- The social worker actively participates in interdisciplinary care plan meetings and coordinates services for safe and timely discharges.
- They provide counseling to residents and families, advocate for residents’ rights, and promote quality of life.
- Acting as a liaison, the social worker fosters communication among residents, families, staff, and outside providers.
- This role involves conflict resolution, maintaining compliant documentation, and helping residents adjust to facility life.
- The social worker compiles social histories, prepares assessments and care plans, and revisits them quarterly or as needed.
- They assist residents and families in understanding care goals and limitations, and educate staff on the psychosocial aspects impacting residents.
- Address and resolve concerns using effective conflict resolution strategies
- Maintain accurate and timely documentation in compliance with regulations
- Collaborate with clinical staff, case managers, and external providers
Requirements
- Minimum of a Bachelor’s degree in Social Work or a related field, Master’s degree in Social Work (MSW) preferred.
- LMSW licensure, preferred
- Experience in sub-acute or skilled nursing settings, strongly preferred.
- Knowledge of Medicare/Medicaid guidelines
- Strong organizational, communication, and interpersonal skills
- Minimum of two years of experience in the social services field, preferably in a nursing facility, hospital or related agency.
- Pleasant personality and ability to work with residents, families, and staff from various departments.
- Proficient in Microsoft Office.
- Ability to work well under pressure and be flexible.
- Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, & sometimes challenging residents
- Work well under pressure & willing to work harmoniously with all staff
