Job Summary
We are seeking a skilled and experienced travel Home Health Physical Therapist to join our team for a 13-week assignment in Hardinsburg, Kentucky. The successful candidate will work closely with patients to develop and implement personalized physical therapy plans, improving their overall health and well-being.
Key Responsibilities:
- Conduct thorough assessments of patients' physical abilities and limitations
- Develop and implement individualized treatment plans to improve mobility, strength, and function
- Provide education and training to patients and caregivers on proper techniques and exercises
- Collaborate with other healthcare professionals to ensure comprehensive care
- Document patient progress and outcomes
Requirements:
- Current licensure as a Physical Therapist (PT) in the state of Kentucky
- Experience working in home health settings
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
