Overview
Skye Hospice & Skye Homecare is seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide high-quality nursing care to patients in their final stages of life, ensuring comfort and dignity. This role requires a strong understanding of patient assessment, health coaching, and the ability to work collaboratively with families and interdisciplinary teams. The Hospice Nurse will play a crucial role in delivering empathetic care while adhering to HIPAA regulations and maintaining patient confidentiality.
Duties
- Conduct thorough patient assessments to determine individual care needs and develop personalized care plans.
- Administer medications and treatments as prescribed, including IV insertion when necessary.
- Provide emotional support and health coaching to patients and their families, guiding them through the hospice process.
- Collaborate with physicians, social workers, and other healthcare professionals to ensure comprehensive patient care.
- Monitor patient conditions regularly, documenting changes in health status and responding appropriately.
- Educate patients and families about end-of-life care options, including pain management and symptom control.
- Maintain compliance with HIPAA regulations and ensure all patient information is handled confidentially.
- Participate in ongoing training and professional development opportunities to enhance skills in hospice care.
Requirements
- Valid nursing license (RN) in the state of practice.
- Experience in hospice or palliative care preferred.
- Knowledge of ICD-10 coding for accurate documentation.
- Familiarity with electronic health record systems such as Axxess is a plus.
- Strong skills in patient assessment, particularly in inpatient settings
- Excellent communication skills, both verbal and written, with the ability to empathize with patients and families during difficult times.
- Ability to work effectively within a multidisciplinary team while demonstrating compassion and professionalism.

Pay: $45.00 - $55.00 per hour
