Job Description and Requirements:

The ideal candidate will have recent experience in home health, preferably within the past year. They should be familiar with electronic medical records (EMR) systems, particularly EPIC, and have proficiency in administering IV medications using Sapphire IVs.
Patient caseload may include individuals with multiple comorbidities, such as CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall-related injuries, infections, cellulitis, post-surgical conditions, wound care, ostomy care, labs, IV infusions, and other complex needs.
