Job Title: Chronic Care Registered Nurse (RN)
">About the Role:
">We are seeking a skilled and experienced RN - Dialysis to join our healthcare team as a Chronic Care Registered Nurse (RN). This is a travel nursing opportunity that requires 12 months or more of Recent Dialysis experience.
">Key Responsibilities:

- Provide and coordinate clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. ">
- Delegate tasks to direct patient care staff, including RNs, LVNs/LPNs, and Patient Care Technicians. ">
- Participate in decision-making, teaching, leadership functions, and quality improvement activities to enhance patient care outcomes and facility operations. ">
- Perform ongoing, systematic collection and analysis of patient data pre, during, and post hemodialysis treatment for assigned patients and document in the patient medical record. ">
- Assess, collaborate, and document patient/family's basic learning needs to provide initial and ongoing education to patients and family. ">
- Direct and provide all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients. ">
- Administer medications as prescribed or in accordance with approved algorithm(s), and document appropriate medical justification and effectiveness. ">
- Serve as a resource for the health care team, participate in staff training, and orientation of new staff as assigned. ">
Requirements:

- Education: Graduate of an accredited School of Nursing. ">
- Experience: 1 year of Nephrology Nursing experience in the last two years as a RN. ">
- Shift Details: Shifts will be discussed in detail during the interview; the role requires a commitment of 40 hours plus overtime and floating to area sites as needed. ">
- Pay Package: Based on 8 hour shifts and 40 hours per week, with tax-free stipend amount to be determined. ">
