Requirements:
- Active CA RN License Required Required for Submission
- Must have recent strong acute care hospital discharge planning experience.
- Conducting patient initial assessments
- Acute Care hospital experience
- EPIC EMR documentation experience
- Strong discharge planning exp REQUIRED
- Required Certifications: AHA BLS
- 50 Miles distance radius rule
