What are the responsibilities and job description for the SEMC RN position at Sleepy Eye Municipal Hospital?
The SEMC Registered Nurse (RN) will play a critical role in delivering comprehensive care coordination, chronic care management, and diabetes education within SEMC. The RN will serve as a Certified Diabetes Care and Education Specialist (CDCES) bringing expertise in diabetes prevention, management, and self-care. The CDCES will educate both patients and staff, foster self-management strategies, and collaborate with healthcare teams to support patients in achieving their personalized health and treatment goals. This dynamic position requires an individual who can effectively manage multiple responsibilities and prioritize tasks in a fast-paced environment. The RN will utilize Medical Staff-approved Condition-Specific Protocols to oversee medication refills, lab work, physicals, and patient visit tracking, ensuring timely and accurate care. In addition to these responsibilities, The RN will work collaboratively with a multidisciplinary healthcare team to ensure seamless patient care while also demonstrating the ability to function independently, taking initiative, and making sound decisions to meet patient needs and program goals.
- Diabetes Education & Chronic Care Management
- Serve as a Certified Diabetes Care and Education Specialist (CDCES), providing education to patients and staff on diabetes prevention, management, and self-care strategies.
- Develop and implement the Chronic Care Management and Primary Care Management programs, ensuring effective patient education and communication between healthcare providers, patients, and families.
- Coordinate and follow up on care plans, promoting continuity of care and helping patients achieve health goals.
- Medication Management & Coordination
- Oversee the Medication Refill Program, ensuring compliance with Condition-Specific Protocols and collaborating with providers for clarifications or adjustments.
- Work closely with administrative and clinical teams to ensure patients are scheduled for and attend necessary appointments and adhere to established follow-up protocols.
- Graduate of an accredited RN program (BSN preferred).
-Must have and maintain a current Minnesota RN License in good standing.
-Certified Diabetes Care and Education Specialist (CDCES) or willingness to obtain certification within 2 years of hire
-preferred 2 years of experience in Diabetes Education
-preferred experience with medication refill process.
