We are seeking a highly motivated and compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary team at Sleepy Eye Rehabilitation Center.
As an LPN, you will play a critical role in delivering high-quality care to our residents, working collaboratively with our Registered Nurses and other healthcare professionals to achieve optimal outcomes.
Key responsibilities include:
- Developing and modifying resident Plans of Care, incorporating evidence-based interventions and prioritizing resident needs.
- Collecting and analyzing resident health data, identifying trends and patterns to inform care decisions.
- Assigning and monitoring nursing tasks and activities, ensuring that unlicensed staff possess the necessary skills and knowledge to provide quality care.
**Requirements**
To be considered for this role, you must have:
- A professional nursing education program approved by the Minnesota Board of Nursing.
- A current Minnesota License as a Practical Nurse.
- Current Certification in Cardiopulmonary Resuscitation (CPR) and emergency care.
