What are the responsibilities and job description for the Speech Pathologist PRN Plus position at SLELS Saint Luke's East Hospital?
Job Description Performs prevention screening evaluation diagnosis management and counseling services to hospital patients with speech language cognitive communication or oral and pharyngeal sensorimotor impairments. Plans directs and conducts habilitative and rehabilitative counseling programs and discharge planning activities. Delegates and instructs support personnel. Employees in this position are responsible to work a minimum of two hard to staff shifts as designated by the Rehab PRN policy and have a minimum work frequency of is six shifts per the entity's posted work schedule of 8 weeks. Behavioral Expectations: Evaluates patients after impairment, injury, or disease. Creates an individualized patient-centered treatment plan based on evidenced based practice and selects appropriate interventions based upon evaluation and assessment of patient. Re-evaluates and provides periodic reassessment of patient progress and contributes to discharge planning. Supervises treatment provided by students and assistants. Participates and makes recommendations for client care interventions to the interdisciplinary health care team. Delegates and instructs supportive personnel in services to be provided. Provides timely documentation per department and/or hospital policy and as required by insurance provider to meet licensing, certification, regulatory, reimbursement requirements, and physician treatment plan. Uses professional judgment to ensure safety to self, patients, and others at all times. Performs all other duties as requested by manager. Participates in quality initiatives Shift 8-4:30PM Must have Weekday Availability (M-F) with some Weekend/ Holiday coverage.
