Role Description
This is a full-time remote role for a Care Coordinator. The Care Coordinator will be responsible for managing patient care, coordinating treatment plans, communicating with patients and healthcare providers, and ensuring continuity of care. Additional tasks include case management, providing customer service, and supporting patients in home care settings.
Qualifications
- Care Coordination and Case Management skills
- Strong Communication skills with patients and healthcare providers
- Experience in Home Care and Customer Service
- Excellent organizational and multitasking abilities
- Ability to work independently and remotely
- Experience in healthcare or a related field is beneficial
- Bachelor's degree in Nursing, Social Work, or related field is preferred
