JOB SUMMARY: Responsible for all required duties in the evaluation and treatment of patients within the department. Completes all necessary documentation required to complete evaluation, treatment, and insurance information on department’s patients.
DUTIES & RESPONSIBILITIES: Performs evaluation and treatment plan on all patients referred for physical therapy. Documents initial evaluation, assessment plan, exercise program, progress notes, and discharge summaries. Fills out any applicable insurance forms needed. Sends evaluations and progress notes/updates through EPIC for the appropriate Medicare Plan of care certifications. Educates patients with explanation of existing problems, treatment being administered and exercise program when appropriate. May be periodically asked to meet on evenings or Saturdays for education in-services and/or continuing education courses. Encouraged to seek and maintain current knowledge and understanding of treatment procedures, continuing education, and participation with sub-committees of SDMG. Completes weekly active patient list (APL) per protocol and reviews department cancellation / no show policy with each patient during their initial evaluation to ensure awareness and compliance with the policy and documents this in the assessment. Performs other related duties as requested by the Rehabilitation Director. May be exposed to hazardous drugs. Attends OSHA training upon initial employee orientation and annually completes an OSHA competency.
RELATIONSHIP WITH OTHERS: Direct contact with patients both in person and over the phone. Must maintain a courteous and effective relationship with patients of the Group. Patient accommodation and satisfaction is critical. Must maintain an effective working relationship with co-workers, staff, physicians and administration.
Requirements
EDUCATION/EXPERIENCE/KNOWLEDGE: B.S. in Physical Therapy necessary, DPT preferred and current New York State license in physical therapy, as well as current registration.
Physical Demands
Lifting: Must be able to lift 20 lbs. infrequently.
Standing: Must be able to stand as needed to assist patient treatment and perform appropriate evaluation.
Sitting: Limited ability to sit required.
