Job Summary: Wellness Director, LPN for Assisted Living and Memory Care is responsible for overseeing the clinical and medical operations within the assisted living and memory care units. The Wellness Director, LPN ensures the highest quality of care for residents, compliance with state and federal regulations, and effective management of the nursing staff. This role works closely with the Assisted Living Manager and Executive Director to achieve the community's healthcare objectives and maintain a safe, supportive environment for residents.

Key Responsibilities:
Clinical Oversight:
- Develop, implement, and evaluate nursing policies, procedures, and care plans.
- Ensure that all resident care is delivered in accordance with professional standards and state regulations.
- Conduct regular assessments of residents’ health and well-being, updating care plans as necessary.
Staff Management:
- Recruit, train, and supervise nursing staff, Caregivers and MedTechs.
- Conduct performance evaluations, provide coaching, and facilitate professional development opportunities.
- Schedule nursing shifts and manage staffing levels to ensure adequate coverage and continuity of care.
Regulatory Compliance:
- Ensure compliance with all local, state, and federal healthcare regulations, including ADHS rules and regulations.
- Prepare for and participate in state and federal surveys and inspections.
- Maintain accurate and up-to-date medical records and documentation.
Resident and Family Relations:
- Serve as a primary point of contact for residents and their families regarding clinical care and health-related concerns.
- Facilitate effective communication between residents, families, healthcare providers, and staff.
- Address and resolve any complaints or issues related to nursing care promptly and professionally.
Quality Assurance and Improvement:
- Implement and monitor quality assurance programs to ensure the highest standards of care.
- Conduct regular audits and reviews of clinical practices, identifying areas for improvement.
- Develop and execute plans for continuous quality improvement in nursing care.
Collaboration and Coordination:
- Work closely with the Assisted Living Manager and Executive Director to align nursing services with overall community objectives.
- Collaborate with other departments, such as activities and dining services, to ensure holistic care for residents.
- Coordinate with external healthcare providers, including physicians and therapy services, to support residents' healthcare needs.
Qualifications:
Education and Experience:
- Bachelor’s degree in nursing (BSN) required; master’s degree in nursing (MSN) or Healthcare Administration preferred.
- Minimum of 3 years of nursing experience.
Licenses and Certifications:
- Current Registered Nurse (RN) license in the state of Arizona.
- CPR and First Aid certification required.
- Certification in Gerontological Nursing (preferred).
Skills and Abilities:
- Strong leadership and management skills with the ability to inspire and guide nursing staff.
- Excellent clinical skills and knowledge of best practices in geriatric and memory care nursing.
- Exceptional communication and interpersonal skills, with a focus on compassionate care.
- Ability to handle complex situations and make critical decisions in a fast-paced environment.
- Proficiency in electronic medical records (EMR) systems and other healthcare technologies.
Work Environment:
- The role is based in an assisted living and memory care facility, with a combination of clinical and administrative duties.
- Occasional evening, weekend, or on-call work may be required to meet the needs of residents and the community.
Other Requirements:
- Ability to respond to emergencies of all kinds.
- Excellent communication skills.
- CPR certified.
- Certified in Assisted Living Management is a plus.
- Ability to handle staffing and employee issues.
- Ability to apply extensive medical knowledge when applicable and appropriate within the guidelines of state and local law.
- Computer literate within the boundaries of job requirements.
- Ability to pass background check.
Physical and Sensory Requirements:

Work Environment:

- On-call availability may be necessary for emergencies or urgent issues.
