Expectation/Requirements:
- Primary focus on patient care
- Accurate, timely documentation
- Case Management experience preferred but not required
- Conflict resolution skills
- Critical thinking skills
- Motivated self-starter with an approachable, friendly, and positive attitude
- OASIS Initial Assessments
- PDGM – Must have worked in this newest payment model
- Professional license – Current and unrestricted
- Strong communication, organizational, and time management skills
- Willingness to learn and help support other team members
- Monthly virtual team meetings
- Ability to attend 2-3 mandatory in-service/skills labs in person per year
- Reimbursement is provided for time and travel
Job Type: Full-time
Pay: $45.00 - $120.00 per hour
