What are the responsibilities and job description for the VBC Pharmacy Technician - Summit Medical Group position at SMG Brand?
Summit Medical Group is searching for a VBC Pharmacy Tech. This is a full time position working Monday - Friday 8:00am - 4:30pm hybrid in office/work from home. This position works under the supervision of The Manager of Integrated Pharmacy as part of a multidisciplinary healthcare team focused on providing care coordination services to identified populations of need.
Examples of Duties (List does not include all duties assigned)
- Under the supervision of the Manager of Integrated Pharmacy, contacts and tracks patient progress regarding medication assistance needs for the purpose of achieving established Care Coordination physician and patient directed goals.
- Understands the intricacies of patient medication assistance programs, as well as third party medication tier structure and formularies.
- Acts as an advocate for an individual's health care needs by identifying and communicating potential needs to the RN or Social Work Team, or Pharmacist.
- Facilitates patient understanding of the physician’s treatment plan, including but not limited to medication adherence, preventive care, and self-management skills.
- Provides assistance resolving concerns about service delivery or provider care.
- Promotes coordination of patient’s primary care provider, specialists, other providers and medication assistance programs facilitating comprehensive, holistic, person-centered approach to care.
- Consults with the Pharmacist and/or other health team members and seek guidance as necessary.
- Participates in establishing and maintaining a therapeutic patient relationship.
- Facilitates and monitors the implementation of services in accordance with the plan of care, promoting high quality integrated care.
- Maintains security and privacy of patients.
- Actively participates in Quality Improvement Activities. Each employee will contribute to the continual evaluation of departmental performance as well as the implementation and measurement of improvement activities that increase the quality of care provided to patients.
- Engages and offers patient’s assistance with medication assistance programs, resources, and services. Resolves patient issues in a timely manner and answers questions to ensure a positive experience.
- Aids patients with access to medications by facilitating refills and/or 90 day prescription fills
- Communicating adverse drug reactions with appropriate healthcare providers.
- Other duties as assigned.
Education
High school diploma or general education diploma (GED) required.
TN Board of Pharmacy Technician Registration required.
Experience
One to three years of experience in a clinical setting, retail pharmacy, or medical office.
Prior experience with disease management, population health management, or other related health care environments (preferred but not required)
Must be PC literate with working knowledge of Windows, word, and excel.
Certification/License
Pharmacy Technician state license (required)
