Full Time Role:
Starting Pay: $43 per hour (Salaried role and pay is based on previous experience)
5 days per week schedule
Part of the On-call manager rotation:
Position Purpose:
The Clinical Rehab Nurse Manager is an RN Nurse who serves in the capacity of a coordinator with assessing, planning, implementation, monitoring and evaluation of the options and services required to meet the residents’ health service needs.

The Clinical Rehab Nurse Manager is an RN Nurse who serves in the capacity of a coordinator with assessing, planning, implementation, monitoring and evaluation of the options and services required to meet the residents’ health service needs.
Primary Job Duties:
- Clinical Rehab Nurse Manager Participates in welcoming new residents and their families to the community.
- RN Initiates and supervises the completion of Physician Certification recertification for rehab stay.
- Manages resident and family concerns in a calm professional manner.
- Serves as a nursing representative for PAN, Bundle Medicare meetings.
- Participates in resident baseline and comprehensive care plan for continuity of care.
- Preparation, review and submission of insurance documentation, authorization, appeals and audits.
- Assessing, implementing, and evaluating resident individual level of care needs and ensuring that is reflected in resident care plan.
- Monitors daily Nursing documentation to ensure resident condition and treatment is being appropriately documented.
- Participates in the Nurse on Call rotation.
- Other duties as assigned.
- Registered Nurse RN license in good standing
- 3 years clinical experience
- Minimum 1 year experience in long term care.
