Social Services Assistant
POSITION PURPOSE: To support the activities of the Social Services department while also assisting with the Admissions processes on an as needed basis
Job Duties Include
Education: Bachelor's Degree - Preferred
Experience: 2 years of relevant LTC experience preferred.

Job Duties Include
- Communicates with resident and families following their admission to the community and ongoingly throughout their stay until discharge.
- Participates in the completion and verification of documentation including but not limited to Advance Directives (i.e., DNR, Living Will, Powers of Attorney), Social History, Pastoral Care Requests, etc. via charts audits
- Collaborates and assists in the admission process when necessary.
- Participates in resident care meetings with families and resident and documents as such.
- Assists in completing the appropriate documentation relating to transitions
- Acts as a reference to resident, families, and staff by providing guided resources and interventions as needed.
- Provides "Active Listening" for Residents/Families directed at problem identification and solution.
- Assists in the discharge process.
- Assists in arranging home health services, private duty and/or equipment as needed.
- Acts as a resource to Families for any areas of concern; redirects to appropriate source as necessary and follows through until resolution.
