- Ensure accurate and timely completion of resident pre-admission screens and updates.
- Conduct lifestyle and care needs assessments to determine initial and ongoing medical and personal care requirements.
- Supervise and train nursing staff in activities of daily living assistance, medication policies, and other relevant areas.
- Serve as liaison between residents, families, and external healthcare providers.
- Develop and implement effective service plans in conjunction with residents, families, and care teams.
**Requirements:**
- LPN licensure in good standing in the state of practice.
- Valid CPR certification.
