Job Summary:
We are seeking a dedicated and experienced Nurse (RN or LPN) to join our team. This role involves managing Medicare A and Medicare Managed Plan residents, ensuring compliance with all relevant regulations, and providing clinical support to the nursing team. Key Responsibilities:
1. Resident Management:
· Oversee the care and management of residents enrolled in Medicare A and Medicare Managed Plans.
· Ensure all care plans are compliant with Medicare regulations and guidelines.
· Conduct regular assessments and reviews of resident care plans to ensure optimal health outcomes.
· Coordinate with interdisciplinary teams to provide comprehensive care to residents.
2. Clinical Support:
· Provide clinical support and guidance to nursing staff.
· Assist in the development and implementation of clinical protocols and procedures.
· Participate in staff training and development programs.
· Serve as a resource for clinical best practices and evidence-based care.
3. Documentation and Reporting:
· Maintain accurate and up-to-date medical records for all residents.
· Prepare and submit required documentation for Medicare A and Medicare Managed Plans.
· Monitor and report on resident health status and care plan effectiveness.
4. Communication:
· Liaise with residents, families, and healthcare providers to ensure clear and effective communication.
· Address any concerns or issues related to resident care promptly and professionally.
· Facilitate family meetings and care conferences as needed.
5. Quality Improvement:
· Participate in quality improvement initiatives to enhance resident care and service delivery.
· Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
· Collaborate with the nursing team to identify areas for improvement and implement solutions.
Qualifications:
· Education: Bachelor's degree in Nursing (BSN) or equivalent; Master's degree in Nursing (MSN) preferred.
· Licensure: Current Registered Nurse (RN) license in [Insert State].
