Job Description :
We are seeking a dedicated and compassionate Primary Care Physician or Nurse Practitioner to join our healthcare team in Bakersfield CA with a
focus on providing comprehensive medical care to the geriatric population. The ideal candidate will be committed to delivering highquality healthcare services and promoting the wellbeing of our elderly patients. While experience with the PACE (Program of AllInclusive Care for the Elderly) program is preferred we welcome candidates with a strong background in geriatrics and a willingness to learn.
Responsibilities :
Conduct thorough medical examinations and assessments of geriatric patients to
establish accurate diagnoses.
Develop and implement personalized care plans considering the unique needs and
conditions of elderly individuals.
Collaborate with interdisciplinary teams including nurses social workers and
other healthcare professionals to ensure holistic and coordinated patient care.
Provide ongoing medical supervision monitoring and adjustments to treatment
plans as necessary.
Educate patients and their families on preventive care treatment options and
lifestyle modifications to enhance overall wellbeing.
Maintain accurate and uptodate electronic medical records documenting patient
history progress and treatment plans.
Participate in regular case conferences and team meetings to discuss patient care
strategies and coordinate services effectively.
Stay informed about advancements in geriatric medicine and contribute to the
continuous improvement of healthcare practices within the organization.
Adhere to ethical and legal standards ensuring compliance with relevant
healthcare regulations.
Qualifications :
Medical degree from an accredited institution and completion of residency in
Internal Medicine or Family Medicine.
Board certification in Internal Medicine or Family Medicine.
Current unrestricted California medical license and DEA registration .
Proven experience working with geriatric patients with a focus on providing patient
centered care.
Familiarity with the PACE program is preferred but not required; willingness to
undergo training if necessary.
Excellent communication and interpersonal skills to engage effectively with elderly
patients and their families.
Ability to collaborate in a teamoriented environment with a patientcentered
approach.
Strong organizational skills and attention to detail.
Commitment to continuous professional development and staying current with best
practices in geriatric medicine.

Only carries a panel of 125 to 135 patients.
They see about 46 patients a day
MonFriday no weekends and no afterhour oncall
