Responsibilities
- Conduct comprehensive assessments to identify client needs and strengths.
- Develop, implement, and monitor individualized service plans.
- Provide support and guidance to clients in navigating social services and community resources.
- Facilitate communication between clients, families, and service providers.
- Maintain accurate documentation of client interactions and progress.
- Advocate for clients’ rights and access to necessary services.
- Collaborate with interdisciplinary teams to ensure holistic care.
- Provide crisis management support as needed.
- Engage in ongoing professional development to stay informed about best practices in case management.
Skills
The successful Case Manager will possess the following skills:
- Strong background in Childcare and Child Welfare practices.
- Experience in Supervising and mentoring individuals or teams.
- Knowledge of Behavioral Health principles and practices.
- Proficient in Crisis Management techniques.
- Background in Social Work or related fields is highly desirable.
- Familiarity with Addiction Counseling strategies.
- Effective use of Motivational Interviewing techniques to engage clients.
- Understanding of Workforce Development initiatives.
