Job Summary: This role is a licensed practical/vocational nurse position that provides direct nursing care to patients in a post-acute care setting on a contract basis. The LPN works closely with other staff, permanent and temporary, and assists them as needed.
Key Responsibilities:
- Contributes to patient assessments by collecting data for analysis by an RN.
- Performs interventions based on the plan of care, including specimen collection and wound care.
- Maintains proper infection control practices and adheres to Infection Control Policies and Procedures and OSHA standards.
- Follows medication pass protocols and meets regulatory requirements.
- Evaluates the effectiveness of nursing care by continually reviewing the nursing care plan.
- Collaborates with the supervising nurse to make revisions and achieve desired outcomes.
- Assists patients with daily living activities, including personal hygiene, elimination, nutrition, and ambulation.
- Advocates for patients' physical and emotional well-being.
- Monitors, documents, and communicates patient status and reactions to interventions with the RN, physician, patient, and families.
- Collects and documents patient information as needed (including weights and vital signs).
- Provides patient and family education on hospitalization, medications, and discharge instructions.
- Ensures accurate and timely hand-off communication.
- Serves as a clinical and administrative resource for nursing services, procedures, aging processes, medications, and outside medical services.
- Supervises CNA's and Caregivers.

Education and Experience:
- Qualified candidates must have graduated from an approved Licensed Practical Nurse program.
- Must possess a current, unencumbered license to practice as a LPN in this state.
- Licensure/Certifications: Current LPN licensed in good standing by their State Board of Nursing.
- Current BLS or CPR/First Aid; Food Handlers Card.
