Job Summary:
The Licensed Practical Nurse (LPN) is a vital member of our care team, responsible for providing comprehensive oversight of resident care, regulatory compliance, and collaboration with interdisciplinary teams.
We are seeking a dedicated and compassionate LPN to join our team. The ideal candidate will have graduated from an accredited Licensed Practical nursing program and possess one or two years of previous acute or geriatric/adolescent or rehabilitation experience.
This role involves conducting patient assessments, collaborating with Registered Nurses (RNs) on care needs, and developing individualized nursing care plans with measurable goals. The LPN will also engage patients and families in care plans and identify discharge needs.
In addition to direct care delivery, the LPN will provide patient care under RN supervision, manage patient health status, recognize changes and emergencies, and follow Whittier Rehabilitation standards. They will also perform basic activities of daily living (ADL) care, transcribe physician orders, and administer medications per policy.
The LPN will accurately document patient information with an understanding of legal implications and demonstrate effective verbal, written, and therapeutic communication with patients, families, and care teams.
Key Responsibilities:
