Position Summary
Summary :
Under the direction of the Director of Social Work, the social worker will provide psychosocial assessment / evaluation of patients admitted to Transitional Care Unit. The Social Worker will provide direct social work services to patients and families, as well as, participate in formulating and implementing discharge plans, identifying unmet psychosocial needs and developing programs to address those needs.
Qualifications
Responsibilities : Education Required
- Must complete in-house orientation program
- Master's in Social Work (MSW) from a school of Social Work accredited by the Council on Social Work Education (CSWE).
