The Social Worker will provide direct social work services to patients and families, as well as, participate in formulating and implementing discharge plans, identifying unmet psychosocial needs and developing programs to address those needs.QualificationsResponsibilities : Education Required
- Must complete in-house orientation program
- Master's in Social Work (MSW) from a school of Social Work accredited by the Council on Social Work Education (CSWE). Experience And Skills
- Excellent interpersonal skills
- Speaks, reads and writes English
- Prior experience in long-term care setting required.
- Knowledge of case management preferred. License / Certifications
