As our Contract Dietitian, you will play a vital role in the Snowline Health team, providing expert nutritional guidance to our patients. Key Responsibilities
- Conduct comprehensive nutrition assessments and develop individualized care plans for patients receiving hospice and palliative care.
- Provide dietary counseling and education to patients, caregivers, and staff to optimize nutritional well-being.
- Collaborate with the interdisciplinary care team to align nutritional plans with overall patient care goals.
- Monitor patient progress and adjust nutrition recommendations as needed.
- Ensure compliance with applicable regulations, policies, and best practices in medical nutrition therapy.
- Document patient interactions and care plans in electronic medical records in a timely manner.
- Stay current with best practices and evidence-based guidelines in nutrition and hospice care
Requirements
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential required.
- Current licensure in the state of California.
- Minimum of two years of experience in clinical nutrition, preferably in hospice, palliative care, or home health settings.
- Strong interpersonal and communication skills to effectively engage with patients, caregivers, and healthcare teams.
- Ability to work independently and manage a flexible schedule based on patient needs.
- Knowledge of nutrition-related concerns in end-of-life care, including symptom management and palliative nutrition strategies.
