Responsibilities: As a Nurse Practitioner (NP) at Snowline Health, you will be responsible for providing compassionate and comprehensive nursing care to our patients and their families. The position is based out of Diamond Springs, CA, and involves a hybrid of on-site and remote work. The role focuses on providing comprehensive, patient-centered primary and palliative care to elderly patients in home-based and clinic settings.
Your duties will include:
Clinical Care We offer ongoing education and training opportunities including our Snowline Scholars Program which offers full-time Registered Nurses, with tuition reimbursement for continuing education relevant to their work within the organization
- Conduct comprehensive health assessments
- Develop and manage individualized treatment plans
- Prescribe medications and treatments
- Provide patient and family education
- Perform necessary procedures within the scope of practice
- Document patient encounters accurately and timely in EMR
- Participate in call coverage rotation
Care Coordination
- Collaborate with interdisciplinary team members
- Serve as the primary point of contact for patients and families
- Coordinate referrals to specialists and community resources
Program Development
- Assist in developing and refining clinical workflows and protocols
- Participate in quality improvement initiatives
- Provide mentorship and training to clinical support staff
Regulatory Compliance and Quality Assurance
- Ensure compliance with state, federal, and organizational regulations
- Participate in internal audits and reviews
- Maintain current knowledge of geriatric and palliative care practices
- Familiarity with CMS billing and coding for home-based care
- Maintain all required professional credentials and continuing education
Community Leadership and Engagement
- Represent Snowline ElderCare in community forums and events
- Act as a resource for staff and community members on elder care best practices
- Demonstrate delivery of high-quality, compassionate home-based care
- Mentor students and other team roles as appropriate
HIPAA Minimum Necessary Access
- Access to clinical client health information
Requirements
- Current valid California license
- Minimum of three years of clinical experience in geriatric, palliative, or primary care
- Proficient in medical terminology, EMRs, and HIPAA compliance
- Knowledge of palliative care and hospice preferred
- Experience in home-based care delivery preferred
- Strong clinical assessment and diagnostic skills
- Excellent interpersonal, communication, and organizational skills
- Ability to work independently and collaboratively as part of an interdisciplinary team
- Valid CA driver’s license with a reliable, insured vehicle for home visits
- Active and updated DEA/CURES credentials
- Knowledge of trauma-informed care principles preferred
