Responsibilities:
· Answer incoming referral office telephone, respond to electronic referral communication and faxes as needed for smooth and timely referral/admission processing
· Provide physicians, patients, family members and other callers with information about Hospice services
· When referral is received obtain initial referral information, coordinate patient evaluation by arranging date and time of meeting with patient and advise admission nurse(s) about appointment time. Provide admission nurse(s) other relevant referral information.
· Notifies referring physician office or agency about need for copies of medical records
· Enter initial referral/admission data into Electronic Medical Record (EMR)
· Create pending file and maintains in alphabetical order for access when needed
· Business day prior to patient scheduled admission voice mail hospice team members to inform they have a new patient
· Following admission make voice mail announcement to all appropriate staff regarding patient admission to service
· Provide admission packets to admission nurse as applicable
· Maintains referral/admission whiteboard in referral office and leads daily “Stand Up” meetings to discuss patients on pending list
· Keep patient-census whiteboards in referral office current
· Keep Residential Care Facility for the Elderly (RCFE) notebooks up-to-date and coordinate distribution
· Keep records of Hospice waivers for RCFEs and notifies admission nurse of presence of waivers before admission
· Notify, via eFax or fax, pharmacy and durable medical equipment (DME) vendor regarding patient initial date of service and contact information
· Per nurse instruction order initial durable medical equipment (DME) and update EMR
· Prepare annual referral and admit report, and documents referral information in Excel tracking log
· Prepare Kaiser Diverted Patient Report, as required fax to Kaiser and maintains file copy for future reference
· Responsible for assuring patient education packets, comfort medication information and intake/admit order documents are assembled and available
· In collaboration with Hospice Clinical Director may create new forms or revise existing forms to meet referral office needs
· Maintains fax machine toner and pater supply for referral office
· Take and relay messages in a timely, clear manner
· Work collaboratively with admission team
· Perform required data entry into the Allscripts system accurately and timely
· Communicate effectively, promptly and professionally with staff, physicians, patients and community referral sources

Requirements
Qualifications:
- High School Graduate or GED
- Four years general office and administrative support experience OR two years general office and administrative support plus an Associate Degree in business or related field
- One year experience in a healthcare setting preferred
- Knowledge of medical terminology
- Demonstrated ability to perform detailed work in a timely manner with accuracy and thoroughness
- Demonstrated ability to prioritize tasks, handle pressure and delicate situations while effectively carrying out job responsibilities
- Demonstrated computer skills with extensive experience using Microsoft Word and Excel required, Access preferred
- Must be able to rotate weekends with the rest of the team
Essential Functions:
