Job Description
Clinical Responsibilities
- Deliver comprehensive, person-centered primary and palliative care services to seniors in home-based and clinic settings.
- Conduct comprehensive health assessments and develop individualized treatment plans.
- Prescribe medications and treatments, provide patient and family education, and perform necessary procedures within the scope of practice.
- Document patient encounters accurately and timely in our Electronic Medical Record (EMR) system.
Collaboration and Communication
Work collaboratively with the multidisciplinary team to address complex health needs, coordinate care, and ensure patient and family satisfaction.
