Family Centered Treatment (FCT) Practitioner

Snowy Peak Community Services is seeking individuals to join our team as a Family Centered Treatment® Practitioner, providing intensive prevention services to families in their home. FCT keeps families together by helping them develop the solutions and skills they need to reduce risks in everyday situations. You could be instrumental in helping families heal, develop new skills, and maintain wholeness. New hires will complete a self-guided online training course and make consistent progress towards achieving FCT Certification from the FCT Foundation through videotaped demonstration of competencies. BASIC PURPOSE OF THE JOB

Exciting opportunity to become certified in an evidence-based program that provides intensive prevention services to families in their home. The FCT Practitioner provides healing and hope to children and families through Family Centered Treatment® program. New hires will complete a self-guided online training courses and make consistent progress toward achieving FCT Certification from the FCT Foundation through videotaped demonstration of competencies. Ability to work from home when not in session with assigned families. Must demonstrate ability to work independently in addition to being an effective team player. Must be able to give and receive feedback to and from your peers.

The mission of the Family Centered Treatment program is to provide professional services to individuals suffering difficulties arising from mental health and substance use related concerns in a holistic framework based upon the values of respect, integrity, and dignity. We strive to communicate these core values by demonstrating empathetic, authentic and genuine care in all of our interactions in meeting the unique needs of the families we serve.

Family Centered Treatment is an evidence based in-home model which utilizes family systems theory to support families who have experienced family dysfunction. Our goal is to maintain children in their homes or reunite children who have experienced out of home placement.

A Family Centered Treatment (FCT) practitioner has an active case load of 4-6 families at a time. You will meet with families multiple times a week in their homes providing FCT sessions. In order to best serve families, we schedule sessions at times that are best for the family – afternoons, evenings, weekends. FCT Practitioners partner closely with other stakeholders of the family.

JOB DETAILS – Family Centered Treatment Practitioner

Job Duties: The duties of a Family Centered Treatment Practitioner may include the following:

· Serve assigned caseloads of up to five families. FCT Practitioner will spend a minimum of four hours per week with a minimum of two weekly contacts per family or as required per the case plan.

· Complete FCT certification within 1 year of start date.

· Attend and participate in weekly team supervision meetings where cases are reviewed, and feedback is provided.

· Complete and maintain all program fidelity documents, reports, and assessments in a timely manner reflecting the FCT language.

· Provides intervention to families in crisis situations.

· Provides FCT in-home family treatment, adhering to and ensuring fidelity to the FCT model.

· Serves as liaison with DHHS, courts, schools, other providers, and community stakeholders.

· Complete and maintain all program fidelity documents in a timely manner.

· Complete reports for court and attend when needed.

· Work closely with at-risk youth and their families, utilizing the Family Centered Treatment (FCT) model.

· Conduct individual and family sessions within the community.

· Provide comprehensive case management to support the mental health and overall well-being of clients.

· Collaborate with interdisciplinary teams to ensure coordinated care and support

MINIMUM EDUCATION, LICENSURE/CERTIFICATIONS, AND JOB REQUIREMENTS

· Minimum of a Bachelor's degree required along with one year of human service work experience. Bachelor’s degree preferred in Social or Family Services, e.g. Social Work, Psychology or Sociology.

· Must have a valid driver's license, acceptable driving record and reliable transportation.

REQUIRED SKILLS, KNOWLEDGE AND ABILITIES

· Mandated Reporter, ability to monitor safety and address safety concerns.

· Demonstrate effective written and verbal communication skills.

· Understanding of child and adolescent development and family systems.

· Flexible to changing situations, ability to work evenings and weekend if needed by the family.

· Able to solve problems, but also able and willing to seek assistance when necessary.

· Works alone effectively, self-motivated, works effectively with others and ability to receive feedback.

· Ability to travel and provide in-home services.

· Home based office option when not in sessions with families.

· Have experience working with families (parents and children)

· Have a trauma-informed perspective

· Work effectively with a culturally diverse population

· Become credentialed as an FCT Practitioner within 12 months of hire

· Be flexible and creative

· Be an enthusiastic learner

· Understand the value of stakeholders and collaboration with community partners

· Be the ultimate team player

· Have conflict resolution skills and willing to embrace conflict
