We are seeking a part-time, weekends only (8 hours total), compassionate and dedicated Licensed Practical Nurse (LPN) to provide high quality nursing care to individuals with intellectual and developmental disabilities (DDA). The LPN will play a critical role in promoting health, safety and well-being of individuals by providing direct nursing services, medication administration, and support for daily living activities. This role requires commitment to person-centered care and the ability to collaborate with interdisciplinary teams, families and caregivers.
Duties and Responsibilities
- Provide nursing care tailored to the unique needs of individuals with DDA.
- Assist individuals with activities of daily living (ADLs), including hygiene, nutrition and mobility.
- Monitor and document changes in health conditions, behaviors and responses to treatments
- Administer prescribed medications and treatments according to physician orders.
- Ensure accurate medication documentation and address potential side effects or adverse reactions
- Conduct routine health assessments and assist in developing individualized care plans.
- collaborate with interdisciplinary teams to evaluate and adjust care plans as needed.
- Educate individuals, families, and staff on health-related topics, medication management, and preventative care.
- Advocate for the rights, health and well-being of individuals with DDA.
- Maintain a safe environment and adhere to infection control and emergency protocols.
- Maintain accurate and up-to-date medical records and ensure compliance with HIPAA and other regulations.
- Count all medication and ensure new MAR forms are correct.
- Check all MAR books in each house weekly to ensure that all transcriptions and documentation is compliant with the state / DDA.
- Order medication refills in a timely manner to ensure that all supported persons have enough medication.
- Ensure that all side effects are listed on the appropriate form.
- Ensure that all Doctors’ orders are in the MAR book for each medication.
- Check to make sure all controlled medication is being counted, filled out correctly and stored in a double locked box, closet or cabinet.
- File medical paperwork in correct format in the Medical Comprehensive Books.
- Works with each house manager to make sure medication training and documentation is completed by all Medication Certified Staff.
- Make sure any medication variances are documented and turned in.
- Performs other job-related responsibilities as assigned.

Part-time position, weekends only and holidays as needed. This position may require occasional travel to manage or assist with other households and / or events. You must have the physical ability to perform tasks related to medication administration.
