Job Summary:
We are seeking compassionate and experienced Home Healthcare Social Workers to join our team at Social@Home. As a Home Healthcare Social Worker, you will play a vital role in enhancing the overall well-being of our clients by providing emotional support, conducting biopsychosocial assessments, and connecting clients to valuable community resources. Your efforts will contribute to creating a positive and enriching environment for those receiving healthcare at home services.
Key Responsibilities:
- Comprehensive Assessments: Conduct thorough biopsychosocial assessments of clients to identify their emotional, social, and environmental needs.
- Care Planning: Collaborate to develop and implement individualized care plans that integrate psychosocial support of the clients and their families.
- Psychosocial Support: Provide emotional support and counseling to clients and their families to help them cope with health-related challenges. Address issues such as anxiety, depression, grief, and family dynamics.
- Resource Referrals: Connect clients to community resources, support groups, and services that address their specific needs and improve their quality of life.
- Care Coordination: Collaborate with healthcare at home providers, including nurses and therapists, to ensure a holistic approach to patient care.
- Client Advocacy: Advocate for clients' rights and needs, ensuring they receive the highest level of care and support.
- Education: Provide education to the clients and families regarding the client’s condition, treatment options, and coping skills. Empower the client and family to actively participate in their care.
- Documentation: Complete and maintain accurate and confidential client records, including assessments, care plans, and progress notes.
- Ethical Practice: Adhere to professional ethical standards, including maintaining patient confidentiality, respecting autonomy, and practicing non-discrimination.
- Cultural Competence: Demonstrate cultural sensitivity and responsiveness to meet clients and their families' diverse backgrounds and needs.
- Communication: Communicate effectively with clients, families, healthcare providers, and community organizations to facilitate seamless care transitions.
- Continuous Learning: Stay informed about current social work practices, healthcare regulations, and community resources to provide the best possible support.
- Self-Care: Prioritize self-care to prevent burnout and maintain effectiveness in providing support to clients and their families.
Qualifications:
- Graduate of an approved social work curriculum and state licensure.
- Previous experience in healthcare, healthcare at home, or a related field is preferred.
- Excellent interpersonal and communication skills.
- Experience with electronic medical records and computer proficiency.
- Strong organizational skills and attention to detail.
- Ability to assess, counsel, and support clients and their families during times of crisis and stress.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to providing person-centered and holistic care.
- 100% travel necessary daily.
