Medical Social Worker needed for full-time, temporary (11 / 18-12 / 30) position in Columbia, Maryland. This is a 9a-5p position.

The ideal candidate will have experience working with aging populations and those facing chronic health issues, offering both emotional support and practical assistance.
Responsibilities :
- Conduct initial psychosocial assessments for new patients to identify emotional, social, and environmental needs.
- Collaborate with healthcare teams, including nursing and therapy staff, to develop comprehensive, patient-centered care plans.
- Provide short-term counseling and crisis intervention to patients and their families, addressing issues related to illness, adjustment, and coping.
- Assist in discharge planning, coordinating services for a smooth transition back home or to other care settings.
- Connect patients and families with community resources, such as housing support, financial assistance, and mental health services.
- Document all interactions and interventions in compliance with facility standards and state / federal regulations.
- Act as a liaison between patients, families, and medical teams to ensure clear communication and holistic care.
- Educate patients and families on topics like advance directives, coping strategies, and community resources.
