Description
Social Worker needed for 1 position in Orange County and 1 position in Rockland County (2 separate positions, 1 for each county) - in-person and home visits required. Schedule : Day Shift (includes a weekend rotation 1-2 weekends / month)

This is a good position for social workers who :
- want to do medical social worker,
- have an LMSW and want to accrue clinical hours with clinical supervision (in order to obtain an LCSW)
- are comfortable with the idea of working with clients and their families in their end of life journey (training will be provided for social workers who are not experienced in hospice work, but interest and comfort to do this work is needed of course).
- live in Rockland or Orange counties (or very close to these counties)

Responsibilities :
Contributes to patient and family counseling and educationParticipates in ongoing interdisciplinary comprehensive assessmentAssesses patient / family psychosocial response to the terminal illness and evaluates coping skillsAssists patients, family, and agency staff in utilizing community resourcesAssists team members in understanding the significant social and emotional factors related to the care of the terminally ill patient and familyCoordinates with team members to plan and implement the hospice plan of careSocial Worker will participate in interdisciplinary team meetingsSocial Worker maintains accurate, thorough and professional written records, and charts relevant information in a timely fashion, following UH documentation standardsParticipates in and is actively involved with the bereavement portion of the program as appropriateParticipates in volunteer and staff education programsDevelops and implements the discharge plan in conjunction with the IDSParticipates in QAPI program and hospice sponsored in-service trainingRefers high risk patientsDevelops / maintains community resource fileIncorporates organization's commitment to customer service and continuous quality improvement into daily work habits. This includes promoting positive co-worker and teamwork relationships and supporting the goals and mission as set forth by the agencyAccepts additional responsibilities as assignedSalary Range : $31 / hr. - 38 / hr. (approx. $56,000-$79,000) and is commensurate with education, applicable experience, licensing and certifications.
Position Requirements
MSW from a graduate school of social work is required. Must be licensed by the State of New YorkLMSW required, LCSW a plusMust have at least two years of supervised experience in the health field or two years of experience in family therapy / counselingSocial Worker in this role must receive certification in hospice and palliative care within three years of hire date and maintain certification during employmentAbility to relate to a diverse population and workforce in a personable and effective mannerAbility to relate to the terminally ill and their families; bereavement experience preferredMust be proficient in MS Office Suite, EMRs and able to learn new software systemsMust have daily access to a vehicle, a valid driver's license / acceptable driving record and insurance coverage for daily travel to patient locationsThis position is currently accepting applications.
