Description Case Manager needed for full-time, temp to hire position in Mercer County.Our Residential Treatment Center provides intensive outpatient services for males ages 9-12 in a 5-bed community home in Trenton. The goal is to support youth reintegration with their family and community through culturally competent, individualized treatment planning.As a Case Manager, you will play a key role in developing and implementing comprehensive case management plans focused on life skills. Responsibilities include facilitating group programming using evidence-based curricula, providing individual and family supportive counseling, and coordinating referrals, advocacy, and linkages. You will participate in service planning, assessments, discharge planning, and case conferences while ensuring compliance with all regulatory and program requirements. The role also involves collaboration with multiple agencies, active participation in team meetings, and accurate documentation and reporting.Position Requirements BSW and at least three years of relevant experience or an unlicensed master's degree with at least one year of related experience.This position is currently accepting applications.
