JOB SUMMARY: The Inpatient Social Worker, in collaboration with the clinical team and medical provider, provides patient and family advocacy, discharge planning coordination, and psychosocial intervention for the high-risk inpatient. The Social Worker strives to promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources among a patient population with complex health needs.
RESPONSIBILITIES:
- Conduct assessments of maternal and child health needs, to identify barriers to treatment and encourage compliance with medical care
- Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care for patients Create plan for care across the continuum, integrating patient/family preferences and values.
- Serve as a resource to assess, evaluate, recommend legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs.
- Advocate for resources and removal of barriers
- Maintain ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
- Collaborates with healthcare providers, community organizations, and families to promote health education, preventive care, and effective health management strategies.
- Educate families about prenatal care, infant care, safe sleep, and Post Partum Mental Health
- Maintain accurate records of patient interactions and outcomes in compliance with hospital policies.
- Provide excellent communication skills with the ability to engage effectively with diverse populations.
REQUIREMENTS:
- Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline.
- Master's in Social Work required.
- 2 years of social work experience in a hospital setting with maternal child health, children and families is required.
- LMSW or LCSW/LCSW-C required.
- MD Social Work License per level of education.
- Demonstrates the ability to follow verbal instructions, as well as the ability to communicate effectively both verbally and in writing.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
- Case Manager credentialed from the American Case Management Association (ACMA) preferred.
