Note :
- Remote to start but needs to be located around Pittsburgh . Field Travel Needed over 50% of the time (Travel will be reimbursable)
- Contract through the end of year with possible contract to hire
- Work schedule Monday-Friday with the weekend work once a quarter
Job Description :
The Community Health Worker (CHW) will be responsible for helping members to navigate and access medical care, community services / resources and develop healthy behaviors. The CHW will act as a caring neighbor to help members address the social, medical and community problems that lead to poor health (SDoH / HRSN). As a priority, interactions will promote, maintain, and improve the health of members and their families. This includes to provide social support and advocate for individuals and community health needs.
