What are the responsibilities and job description for the Infusion Therapy Coordinator position at Software Methods, Incorporated?
Software Methods, Incorporated is seeking a full-time Clinical Care Coordinator to support our client in the Media, PA area. This is a 100% on-site position.
The Clinical Care Coordinator will be responsible for providing high-quality care coordination services for complex members receiving medication in home infusion or in-office infusion settings.
- Directly manage Clinical Care Coordinator work for complex members.
- Conduct thorough research on available programs for infused medications.
- Establish and maintain relationships with participants, foundations, and providers during transitions.
- Collaborate with providers to ensure they order medications through manufacturer programs and work with TPA for replacement drug setup.
- Improve the use of telecommunications and IT services to enhance efficiencies.
- Document all interactions with members, healthcare providers, pharmacies, and financial assistance entities in the Customer Relationship Manager software in a timely manner.
- Regularly review pending activities/events and customer requests to provide excellent customer service and experience.
- Demonstrate expertise in Payer Matrix patient database, including data elements, case statuses, outcomes, documentation requirements, and company metrics.
- Identify available financial assistance programs for specialty drugs prescribed for active members.
- Report financial assistance program trends to supervisors.
- Establish effective communication with member's health plan sponsors, funds, or PBMs as needed.
Requirements:
- Master's degree in Pharmacy; specialty pharmacy, home infusion, or physician infusion experience preferred.
- Minimum 3 years customer service experience.
- Exceptional written and oral communication skills.
- Ability to multitask and handle consistent workflow.
- Strong time management and prioritization skills.
- Proficiency in computer, email, and MS Office applications.
