Ideal candidates will have a strong commitment to enhancing patient outcomes and maintaining excellent communication with care teams and family members.Salary : DOE per yearEssential Duties and Responsibilities : Provide high-quality therapy services adhering to the Standards of Practice.Conduct thorough evaluations to develop patient-specific plans including both short and long-term goals.Monitor and report patient progress, adjusting treatment plans as necessary.Educate patients, caregivers, and healthcare team members to ensure effective treatment and care continuity.Maintain regular communication with physicians and healthcare team members to coordinate optimal care.Strategically plan for patient discharges setting realistic goals to ensure sustained recovery.Prepare detailed reports for the Clinical Director regarding significant patient or family issues.Minimum Position Qualifications : Valid state licensure as a Physical Therapist.Current CPR certification.Valid driver’s license and automobile liability insurance.Proficient knowledge of therapy practices and equipment.Strong documentation and communication skills.At least one year of experience in a physical therapy role.Ability to establish and maintain effective working relationships with patients, families, and healthcare team members.Preferred Qualifications : Experience in home health care.Advanced certification in physical therapy or a specialty area.Proficient in using electronic health records and telehealth platforms.Skills : Excellent patient care and assessment skills.Strong interpersonal and communication skills.Ability to work autonomously in a home health setting.Proficiency in the latest physical therapy techniques and tools.Strong problem-solving skills with the ability to adapt to changing environments.#J-18808-Ljbffr
