Your Role in Transforming Care:
As a valued member of our healthcare team, you will deliver essential psychosocial support to both in-clinic and home dialysis patients. By leveraging social work strategies, you will help patients adapt to their treatments, improve their quality of life, and support families in navigating the challenges of chronic kidney disease.
Key Responsibilities:
- Conduct comprehensive assessments to understand patients' emotional, psychological, and social needs
- Collaborate with the healthcare team to develop personalized care plans that promote optimal outcomes
- Provide ongoing counseling and emotional support for patients and their families
- Offer education to help patients understand their condition, treatment options, and self-care strategies
- Assist patients in connecting with vital resources such as home health services, vocational programs, or financial assistance
- Support transplant referrals and provide guidance throughout the transplant process
- Educate patients on advance directives and end-of-life planning to ensure informed decision-making
- Advocate for patients' rights while ensuring their understanding of facility policies, grievance procedures, and available support services
What We’re Looking For:
- Education: Master's degree in Social Work (MSW)
-Licensure: Valid state social work license, meeting local practice requirements
- Experience: Entry-level candidates with 0-2 years of related experience are welcome
- Skills: Strong communication, empathy, and problem-solving skills are essential
