Job Summary
SojouRN is seeking an experienced Physical Therapist to provide high-quality Physical Therapy services to patients in their homes. This role offers a unique opportunity to work in a dynamic home health setting, collaborating with patients and caregivers to achieve optimal outcomes.
Key Responsibilities:
- Conduct thorough Physical Therapy assessments to identify patient needs and develop tailored treatment plans.
- Document and report patient progress, including reactions to treatment and changes in condition.
- Provide education and training to patients and caregivers on therapy programs and device use, promoting independence and physical/cognitive functioning.
- Supervise Physical Therapy assistants and certified nursing assistants as needed, ensuring exceptional care delivery.
Work Schedule
Monday-Friday day shift schedule with flexible start and end times based on patient needs.
One weekend every six weeks (Saturday and Sunday day shift) and two holidays per year on a rotating basis.
No on-call responsibilities.
