At SojouRN, we're seeking a dedicated and compassionate Home Health Physical Therapist to join our team. As a Physical Therapist in our Home Health team, you'll play a vital role in providing high-quality Physical Therapy services to patients in their homes.
Responsibilities:
- Conduct comprehensive assessments of patient functioning levels and develop personalized treatment programs to improve independence and physical/cognitive functioning.
- Provide expert guidance and education to patients and caregivers on therapy programs and device use.
- Supervise physical therapy assistants and certified nursing assistants as needed, ensuring seamless delivery of care.
- Maintain accurate records and reports on patient progress, facilitating effective communication with the healthcare team.
Schedule: Monday-Friday; day shift (start & end times vary based on the patient needs) 1 weekend every 6 weeks (Saturday & Sunday day shift w/scheduled patients) & 2 holidays per year (1 summer, 1 winter on a rotating basis); no on-call.
