We are seeking a qualified Home Health Physical Therapist to join our team in various locations, including Addison, IL; Antioch, IL; Barrington, IL; and more.
Responsibilities
- Perform comprehensive Physical Therapy assessments and develop personalized treatment programs for patients in their homes.
- Record and report patient progress, reactions to treatment, and condition changes.
- Instruct patients and caregivers on therapy programs and device use.
- Supervise physical therapy assistants and certified nursing assistants as needed.
- Prepare detailed reports and participate in staff development activities.
Schedule and Shift Details:
- Monday-Friday, day shift with varying start and end times based on patient needs.
- One weekend every six weeks (Saturday and Sunday, day shift) and two holidays per year.
- No on-call shifts required.
Requirements:
- Masters degree or higher in Physical Therapy.
- Valid Physical Therapist license in the state of Illinois.
- Valid driver's license.
