We are seeking an Occupational Therapist to provide in-home therapy services to patients. The ideal candidate will have a Bachelor's degree or higher and at least one year of experience in Occupational Medicine or two years of experience in Social Work.
The Occupational Therapist will be responsible for performing assessments, developing treatment plans, and implementing therapeutic programs. They will also provide guidance and instruction to patients and family members on adaptive and progressive approaches.
Key Responsibilities:
- Perform Occupational Therapy assessments and develop short and long-term goals.
- Provide guidance and instruction to patients and family members on therapeutic procedures and activities.
- Instruct patients and family members on the use of orthotic or prosthetic equipment.
- Conduct functional and cognitive re-evaluations as needed.
- Maintain clinical and progress notes and submit required paperwork in a timely manner.
- Report to the physician on patient progress and response.
- Arrange for outpatient occupational therapy services as necessary.
- Provide consultation to other members of the home health team.
- Prepare periodic reports on care being provided.
- Maintain and update professional competency through in-service attendance and similar resources.
Schedule and Shift Details:
- Schedule: Full Time (40 hours/week, Monday-Friday, 8-5 PM)
