Required Qualifications:
Master's degree or higher
Healthcare experience (1 years)
Valid Physical Therapist license in the state of Illinois (IL)
Valid Regular Driver License (DL)
As a Physical Therapist on our client's Home Health team, you will provide Physical Therapy services to patients in their homes. Your responsibilities will include assessing patient functioning levels, developing treatment programs, and instructing patients and caregivers in therapy programs and device use. You will also record and report patient progress, supervise physical therapy assistants and certified nursing assistants, and prepare reports.
Schedule Details:
- Monday-Friday; day shift with varied start and end times based on patient needs
- 1 weekend every 6 weeks (Saturday & Sunday day shift with scheduled patients)
- 2 holidays per year (1 summer, 1 winter on a rotating basis); no on-call
You will perform local travel to residential homes as needed.
