Job Summary:
We are seeking a dedicated Registered Nurse (RN) to join our Home Health team at Sojourn. As a key member of our team, you will be responsible for providing high-quality patient care in the comfort of their own homes.
Responsibilities:
- Provide Home Health care to patients in the local area
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
- Conduct thorough assessments and evaluations to identify patient needs
- Develop and implement individualized care plans to meet patient goals and outcomes
- Monitor patient progress and adjust care plans as needed
- Communicate effectively with patients, families, and healthcare teams
Requirements:
- 1 year of professional experience as an RN (Home Health experience preferred)
- Associates Degree (or higher) in Nursing
- BLS and CPR Certification
- Able to work full-time schedule: Monday – Friday 8a-5p, 1 on-call shift a week (5p-10p), and 1 weekend a month
- Travel Position (Travel Reimbursement provided)
