What are the responsibilities and job description for the FT Clinical Leader Assistant position at Solaris Hospice?

Responsibilities:
· Reviews available patient information related to the case, including home visits, to determine hospice/home care needs. Helps assign appropriate hospice personnel to the case as needed.
· Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course and reports findings to the Clinical Leader as requested.
· Assists Clinical Leader in establishing immediate and long-term therapeutic goals and in setting priorities.
· Conducts record reviews and communicates findings and recommendations to the Clinical Leader.
· Assists in the screening and interviewing process of new employees when designated and makes recommendations for employment of individuals.
· Assists Clinical Leader and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range for Clinical Leader review.
· Complies with accepted hospice standards and principles.
· Promotes customer service orientation to all hospice personnel.
· Helps assist in quality assessment performance improvement teams and activities.
· Performs other duties and activities as delegated by the Clinical Leader.
Qualifications and Requirements:
· Must be a high school graduate or GED
· Previous experience in hospice/home care setting.
· Complies with accepted hospice standards and practice.
· Have excellent observation, good judgment and communication skills. Understands hospice philosophy and issues of death/dying.
· Understands principles of hospice.
· Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
